Memory Shards are one of The Medium’s many collectibles. These are moments frozen in time that provide some backstory, and they appear when you’re simultaneously bridging the real and spirit worlds. In the spirit realm, you will notice white flutterings in the air. By inspecting nearby items, you can reconstruct the memory with the left thumbstick to piece together what happened here in the past.
Finding all 10 unlocks the Puzzling Out the Past achievement, which is worth 40 Gamerscore. However, the achievement requires you to locate them all in a single playthrough, so you will need to know their whereabouts as you play, so you don’t miss any.
William Blake book
The first Memory Shard can be found after you drop down a hole in the floor in the Niwa Hotel. Marianne will enter a room that she describes as cold, and you need to find a special item to power a Spirit Well to remove a moth swarm blocking your exit. Before beginning this side quest, head to the back left of the room to find a book by William Blake. Interact with it to start the Memory Shard.
Stacked boxes
From the above location, head outside onto the balcony. Some rubble will fall that prevents Marianne from progressing in the real world. Use her Out of Body Experience ability to slide along the gap to the next room. You will see the Memory Shard, but you cannot access it until Marianne’s real-world self is present.
Head to the back right of the room and look through an opening on a door. After the jumpscare, drain the blood-filled bath and grab the razor. Return to your body and use the razor to cut the skin door to your left. Head back around to the Memory Shard and interact with the stacked boxes in the real world.
Hospital drip
In the Dayroom Center’s main area following a cutscene with Sadness. Walk straight ahead and inspect the drip.
Piano keys
After you complete the globe puzzle, which allows you to unblock the door to an outside area, follow the path around into another room. Look for some piano keys on a table on the left and interact with them.
Glasses
Inside the teacher’s room after you obtain the key. Head towards the wall with a flower and butterfly mural to unlock the door. Walk to the desk at the back and inspect the glasses.
Painting
Once you progress past the moth swarm using Spirit Shield, head downstairs and walk across the room to the right. Interact with the painting that is sitting on an easel.
Toy doll
Follow the spirit trail from the painting to a toy doll sitting on an overturned chair. Inspect it.
Ribbon
Follow the spirit trail from the toy doll to a ribbon on a desk on the left side, and interact with it.
Fuel container
Outside the Red House. Before heading towards it, walk to the left to see plenty of debris. There is a fuel container in the rubble. Inspect it to bring up a Memory Shard.
Gas Canister
The final Memory Shard is outside the entrance to the Red House. It’s on your left, and you need to inspect it to trigger the next cutscene.
